Regis College Sports Medicine Forms

The Regis College Athletic Department seeks to provide the best possible care for our student-athletes. Any information gathered, on both student-athlete and/or guardian, is considered confidential medical information. This information is part of the student-athlete's medical record and will help expedite medical care in the case of an illness or injury. 

All student-athletes must be cleared through the Sports Medicine Clearance process, completed yearly through the Regis College PrivIT on-line record keeping system. In addition, student-athletes will undergo baseline concussion screening protocol prior to participating in any organized athletic activity.

The Sports Medicine Department requests that this information is returned by the following deadlines:

  • Fall AthletesJuly 1st  
  • Winter and SpringAugust 1st 

Delayed submission will affect your participation status and may affect your invitation to pre-season or ability to participate in team workouts or non-traditional practices.

Pre-Participation Exams/Physicals

The NCAA requires student-athletes who are beginning their initial season of eligibility to undergo a medical examination or evaluation. All first-year, transfer, and student-athletes that have not previously participated must have the examination or evaluation administered within six months prior to participation in any practice, competition or out-of-season conditioning activities.

The Sports Medicine Department requires that all first-year, transfer, third year, fifth year, and any student-athletes who have not previously participated in Regis College Varsity Athletics undergo a physical examination prior to participation. It is strongly recommended that all student-athletes undergo a physical examination every year.

ALL PRE-PARTICIPATION PHYSICALS MUST BE ON REGIS SPORTS MEDICINE PAPERWORK. NO OTHER PHYSICAL FORM WILL BE ACCEPTED AS PROOF OF PHYSICAL.

Sickle Cell Information

The NCAA is requiring that all NCAA student-athletes have knowledge of their sickle cell trait status. This testing is usually done at birth and records can be obtained on-line by searching for newborn screening results in your state of birth or by calling your state of birth's screening lab.

All student-athletes participating in their first year must either:

1. Provide proof of prior sickle cell testing and have them faxed directly to Regis Sports Medicine (781-768-8329) or upload directly to PrivIT

2. Complete testing through Regis College Student Health Services or with their individual physician.

ADHD/ADD Medication

The NCAA requires any student-athlete who takes ADHD or ADD medication to report all pertinent information to the Regis Sports Medicine staff. Required information includes doctors' diagnostic evaluations (see link to sample below) and doctors' prescription of medication.

A medical exception document or updated letter from a prescribing physician must be submitted yearly or if there have been any changes in the prescribed medication or dosage.

Medical Exception Information

Effective August 2009 the NCAA will institute stricter guidelines for eligibility under medical exceptions for banned substances, including stimulants used to treat ADHD/ADD, medications for male-pattern baldness, peptide hormones (hGH), and anabolic steroids (testosterone) or any other medications banned* by the NCAA.  There are no medical exceptions for street drugs. 

2022-23 NCAA BANNED SUBSTANCES LIST

The NCAA bans classes of drugs* because they can harm student-athletes and can create an unfair advantage in competition. Some legitimate medications contain NCAA banned substances, and student-athletes may need to use these medicines to support their academics and their general health. The NCAA has a procedure to review and approve legitimate use of medications that contain NCAA banned substances through a medical exceptions procedure. The benefit of a medical exception  procedure  is  that  in  most  cases  the  student-athlete’s  eligibility  remains  intact  during  the process. Any student-athlete who tests positive from the effective date will need to comply with this stricter application, even if that student-athlete had received an exception for the use of stimulant medication prior to August 2009.


Concussion Screening

All student-athletes perform baseline concussion screening prior to athletic participation. This is a three-step process that will take place after completion of the PrivIT Sports Medicine clearance process utilizing Impact computer testing, SCAT 5 interview and SWAY balance screening.


Regis College Requirements

Immunization and Health Records required by the university should be uploaded to the CastleBranch system, this is separate from the PrivIT system and both must be completed. Information on that process can be found here:
